1. General Information

Under the Law No. 6698 on the Protection of Personal Data (“KVKK”), data subjects (“Applicant”) are granted the right to make requests concerning the processing of their personal data pursuant to Article 11 of the KVKK.

This form has been prepared in order to identify your relationship with Mateks Makina ve Yedek Parça Sanayi – Dursun Bal, to determine in full the personal data processed by Mateks Makina, to respond to your application accurately and within the statutory period, and to facilitate your application process.

In order to ensure the security of your personal data and to prevent unlawful data transfers, Mateks Makina may request additional information for the purpose of verifying identity and authority. Where the information provided by the Applicant is inaccurate or not up to date, or where the requests are made without authority, the responsibility in this regard lies with the Applicant.

Pursuant to Article 7 of the Communiqué on the Procedures and Principles of Application to the Data Controller, where the Applicant’s request is answered in writing, no fee is charged for up to ten pages. A processing fee of 1 Turkish Lira may be charged for each page exceeding ten pages. Where the response is provided on a recording medium such as a CD or flash drive, a fee may be requested equal to the cost of that medium.

2. Scope of the Right of Application under Article 11 of the KVKK

The Applicant may apply to Mateks Makina and make requests on the following matters:

1) to learn whether their personal data is processed,
2) to request information if their personal data has been processed,
3) to learn the purpose of processing and whether the data is used in accordance with that purpose,
4) to learn the third parties to whom their personal data is transferred domestically or abroad,
5) to request rectification of their personal data where it has been processed incompletely or inaccurately, and to request that the action taken in this regard be notified to the third parties to whom the data was transferred,
6) to request erasure, destruction or anonymisation of their personal data where the grounds requiring its processing have ceased to exist, even though it has been processed in accordance with the KVKK and other applicable legislation, and to request that the action taken in this regard be notified to the third parties to whom the data was transferred,
7) to object to a result arising against them from analysis of the processed data carried out exclusively by automated systems,
8) to claim compensation for damage arising from the unlawful processing of their personal data.

3. Method of Application

Pursuant to Article 13/1 of the KVKK, applications concerning these rights must be submitted to us in writing and signed, or by the other methods determined by the Personal Data Protection Board.

Written applications may be submitted by printing out the form and delivering it:

– in person by the Applicant,
– through a notary public,
– by registered electronic mail (KEP) address, secure electronic signature or mobile signature, or
– by e-mail sent from the e-mail address previously notified to us by the Applicant and registered in our system.

Mateks Makina may request additional information and take the necessary measures to verify the identity of the Applicant in applications made by e-mail.

Pursuant to Article 13/2 of the KVKK, your application will be answered in writing or electronically within thirty days from the date of its notification or receipt, depending on the nature of the request.
